B.Information on Registration 注意事項：
1. New S1 students allocated to the school have to be attended with their parents/guardians to register at the school designated.
派往本校的中一新生須由家長 / 監護人陪同前往本校註冊。
2. New S1 students are required to bring the following documents:
中一新生須帶備以下文件：
(a) Hong Kong Personal Identity Card 身份証
(b) Allocation Slip 派位證
(c) School Reports of Primary 5 and 6 (original and copy)
小五及小六成績表 (正本及影印本)
(d) One personal photo 相片一張
(e) Proof of Address (e.g. copy of electricity bill or other)
住址證明 (如電費單或其他)
3.All registered students must attend the Pre-Secondary 1 Attainment Test on 14 July (Tuesday) from 8:30 to 12:30. Students must wear primary school uniforms and bring the Allocation Slip to attend the Test. If they cannot come, they must inform the school office with a letter signed by their parents on or before 13 July (Monday), stating the reason of absence.
所有中一新生須於7月14日 (星期二) 上午8:30至下午12:30參加中一入學前香港學科測驗，新生須穿著小學校服，並帶備中學學位分配派位證應考。新生如因要事未能參加測驗，學生家長須於7月13日 (星期一) 或以前呈交請假信到校務處。

1. 此表格收集之個人資料只用作加拿大神召會嘉智中學之中一收生用途。
2. 所有申請資料絕對保密。
3. 申請表會於收生完成後一年内盡快銷毀。
